Micropenis is a rare congenital condition affecting about 2% of men, seen in approximately 1 in 1000000. Due to hormonal or developmental issues, the penis fails to grow to normal size, with the erect length measuring less than 3 inches.
A young man born with underdeveloped testes, low testosterone underwent a transformative surgical procedure at Medicover Hospital, Navi Mumbai. From a penile length of under 2.5 inches, doctors helped him achieve a functional erect length of 6 inches, giving him hope for a new future
In a first-of-its-kind case in Navi Mumbai, a team led by Dr Sanish Shringarpure, Urologist and Andrologist, along with Dr Ashish Sangvikar, Medicover Hospital, successfully performed a penile lengthening surgery on a 26-year-old man with a congenital condition of micropenis. The patient had very low testosterone levels (120 ng/dl), small, underdeveloped testes, and high FSH and LH levels, indicating hormonal imbalance and infertility. He desired a penile length sufficient for vaginal penetration and to lead a fulfilling married life. Doctors at Medicover performed a combination of suspensory ligament release, V–Y advancement plasty, and suprapubic liposuction. At the end of a two-hour procedure, the man had achieved an erect penile length of 6 inches, a remarkable surgical success.
Since puberty, Nitin Kumar (name changed), a resident of Alibaug, had lived under the heavy shadow of shame, isolation, and deep emotional pain. Born with a micropenis and underdeveloped testis, he silently carried a burden he could never speak about, not even with his closest friends or family. Every time he looked in the mirror, he questioned his worth as a man. “Am I enough? Will anyone ever accept me?”. What if I’m judged? What if they make fun of me? Will I be ridiculed and shamed forever? How am I supposed to live with that?”. These haunting questions echoed in his mind daily and haunted him constantly. The fear of being mocked, rejected, or misunderstood kept him away from friendships, romantic relationships, and even basic medical help. He avoided changing rooms, social events, and often distanced himself from people just to hide his truth. As marriage talks began in his family, the pressure grew unbearable. “I couldn’t even think of intimacy,” he said. “The thought that I could never satisfy a partner would pressure me every day.”
His confidence shattered with every passing year. But it was his deep-rooted desire to love, to be loved, to be a father someday that finally pushed him to seek help. The patient presented with a micropenis of 2.5 inches, linked to complex hormonal and developmental issues. We did his genetic testing and hormonal profile. Micro penis is a congenital anomaly found in 2% of men, 1 in 1000000, where, due to some developmental defect or hormonal defect, the penis doesn’t grow to full size, and the erect penis length is less than 3 inches. These patients often live in silent agony, avoiding even something as routine as using a public urinal out of deep fear of being judged. The constant anxiety erodes their confidence, making them withdraw from relationships and even delay marriage. They carry a heavy emotional burden.”
Dr Sanish Shringarpure further added, “Our approach was to address the penile length for functional and psychological well-being. We performed a release of the suspensory ligament, which helped bring the penis outward, added V–Y plasty to extend the skin, and did suprapubic liposuction to enhance visibility and effective length. The result, an erect length of 6 inches, was beyond expected. The uneventful procedure lasted for 3 hours. Most importantly, the patient left the OT with hope and a smile, and that matters most. The patient was discharged 4 days post-surgery. There is still a lack of awareness in society about these procedures. However, we have started getting inquiries for this procedure now. Micro penis is a congenital condition, but via surgery, it is possible to provide penile length sufficient for vaginal penetration and satisfactory intercourse. Men with both micropenis and those with average penis size seeking further enhancement are inquiring about this procedure.”
“If this condition is diagnosed in early childhood, then hormonal therapy can help improve the penile length. But most of these patients come for treatment in early adulthood, especially when they are looking for marriage. Then, surgery remains the only option. There are various types of surgeries described to improve penile length. Mostly, surgeons use a combination of different procedures to achieve maximum increment in length. Post-surgery, most patients can have a very satisfactory sexual life,” underscored Dr Ashish Sangvikar, Plastic and Cosmetic Surgeon.
