Mrs Riya, a resident of Lullanagar, had been struggling with severe endometriosis symptoms for five years, experiencing excruciating pain during the first couple of days of her menstrual cycle. This condition had significantly affected her daily life and fertility. Seeking help at Motherhood Hospitals, Riya was diagnosed with bilateral ovarian cysts and adhesions, complications commonly associated with endometriosis.

Endometriosis is a challenging condition characterized by the growth of tissues similar to the lining of the uterus outside of it. These abnormal tissues can implant on various organs, leading to symptoms such as painful periods, pelvic pain, and infertility. Fortunately, endometriosis is treatable, and many women can achieve their fertility goals with appropriate care. In India, 6-10 % of women suffer from endometriosis. The incidence of infertility in these women is 5-6 times higher than the general pollution. On the other hand, the chances of endometriosis in a patient presenting with infertility is a whopping 40-50 %

Upon examination, Dr. Padma Srivastava discovered that Riya had developed bilateral ovarian cysts, measuring approximately 8 cm along with significant adhesions. Adhesions are abnormal bands of fibrous tissue that can form between organs, causing pain and infertility. These findings were critical in understanding Riya’s challenges with conceiving.

To address these issues, Dr. Padma Srivastava performed a laparoscopic cystectomy, a minimally invasive procedure to remove the cysts while preserving Riya’s ovaries. Additionally, the adhesions were meticulously released to enhance pelvic anatomy and improve fertility prospects.

Given Riya’s infertility history, Dr. Padma Srivastava recommended Intrauterine Insemination (IUI) as a fertility treatment option. IUI involves placing washed and concentrated sperm directly into the uterus during ovulation, thereby increasing the chances of successful fertilization.

Dr. Padma Srivastava explained, “Endometriosis has become increasingly prevalent in recent years. Previously 10 years back till now, endometriosis cases have seen a drastic rise, particularly in those aged 24-29. On average, we encounter 2-3 cases per week. The delay in pregnancies is one contributing factor to this increase. While endometriosis can significantly impact fertility, many women can successfully conceive with appropriate treatment. And annual gyne checks play a significant role in it. In Riya’s case, laparoscopic surgery and subsequent IUI were instrumental in overcoming the challenges posed by her condition.

Dr. Srivastava further emphasized, “Women with endometriosis can conceive naturally, although infertility is more common. While IVF is often considered, it’s not always necessary. Fertility-enhancing laparoscopic surgery can restore pelvic anatomy and improve ovarian function. Approximately 25-30% of patients with endometriosis conceive with medication alone.”

To assess ovarian reserve, an Anti-Müllerian hormone (AMH) test is essential. This test measures the remaining egg supply, which is crucial for understanding fertility potential. AMH testing is particularly recommended for women over 35, those with a history of infertility, and those considering fertility treatments, and even young women who want to understand their fertility window.
