In a remarkable surgical achievement, a 19-year-old female patient, Priyanka, underwent a successful operation at NIIMS Hospital, Greater Noida, to remove a massive 3.8 kg ovarian cyst. The cyst, measuring approximately 28x20x10 cm, was identified after the patient presented with severe abdominal discomfort and visible swelling.

Initial diagnostic scans had suggested the mass originated from the mesentery of the bowel, raising concerns of a potential malignancy. After extensive evaluation, cancer was ruled out. The case was then taken up for Exploratory Laparotomy under Dr. Harsh Bhati, Consultant General Surgeon, and his surgical team.

Dr. Harsh Bhati, Dr. Shehwar Koul, Dr. Partha Nandi, and Dr. Vivek performed the 1.5-hour complex surgery. Dr. Ayushmann and Dr. Surekha expertly managed anesthesia support. Upon exploration, the team discovered and successfully excised a left-sided para-ovarian cyst.

Dr. Harsh Bhati, Consultant General Surgeon, NIIMS Hospital, said, “The case was a diagnostic dilemma to begin with. Despite the size, the patient was mostly symptomless. Even CECT W/A was suggestive of a Mesenteric / Peritoneal cyst. So we started the case with a different approach. While performing surgery, we noticed that the cyst was arising from the left paraovarian tissue and extending from the liver to the uterus. That made the case more challenging as we had to save the ovary in such a young patient. Dissection was difficult and tricky, and took longer than expected.”

Postoperatively, the patient is recovering well under observation, with no complications reported. The timely diagnosis, multidisciplinary approach, and skilled surgical intervention played a critical role in the successful outcome.
