A 40-year-old woman with complaints of excessive bleeding, anaemia and multiple fibroids in uterus, patient is discharged in stable condition after open myomectomy procedure

New Delhi, 31st August 2026: Almost for 15 years, a lady aged 40 years had been struggling to conceive a child. Even though she had tried several options including IVF, medication, and injections in her country (The Republic of Congo, a country in Central Africa), she was not able to conceive. Besides, the lady had suffered from severe periods and abdominal pains.

As her situation deteriorated with continued bleeding for almost 20 days along with fever, she went to India for further treatment. The tests conducted at Apollo Spectra Hospital in Delhi showed that there were around 13 fibroids in and around her uterus, with the largest being about 7.5 cm.

Her uterus had enlarged due to fibroids to become almost as big as that of someone four months pregnant. She was anaemic since, as a result of continuous bleeding, her haemoglobin level had reduced to 6.8 g/dL.

Dr. Megha Mittal, Senior Consultant in Obstetrics & Gynaecology, conducted the operation at Apollo Spectra Hospital, Delhi. She successfully removed all 13 fibroids in one go, without removing the uterus. It was especially important for the patient as she wanted to get pregnant further on.

“Being a complex case due to multiple big fibroids, very severe anaemia, obesity, and her longstanding history of infertility, our first goal was to excise all the fibroids from the uterine cavity without harming her uterus and fertility. With her haemoglobin being just 6.8 g/dL, maintaining her general well-being before and during the surgery was another priority for us,” said Dr. Megha Mittal, Senior Consultant – Obstetrics & Gynaecology, Apollo Spectra Hospital, Delhi.

Prior to the operation, her condition was optimized through the management of the anaemia, bleeding, and fever. Due to the complexity of the procedure and her anaemia, strict management of blood and fluids was done. Prior to the procedure, she received three units of packed red blood cells, four units of fresh frozen plasma, and one unit of packed red blood cells during the procedure, and lastly, two more units of packed red blood cells post-procedure.

The operation lasted two and a half hours. All thirteen fibroids were successfully excised, and the uterus was then stitched back and saved. The patient was in good condition during the surgery, with no serious complications recorded either before or during surgery.

“For a woman who has tried for conception for nearly 15 years, preservation of the uterus played an essential role in ensuring her options for reproduction. Although surgery does not provide a surety of conception, the removal of fibroids successfully has ensured that the journey continues. In this case, the preservation of the uterus meant more than just performing the surgery; it meant ensuring the possibility of conception. The journey for conception can now proceed ahead, subject to her recovery and subsequent medical assessment,” further said Dr. Megha Mittal.

The patient made a good recovery and was released in a stable condition. She is in good health at the moment. The embryos have been cryopreserved already, and after an approximate recovery period of three to four months, she can be considered for embryo transfer.

Dr. Mittal advised women not to ignore symptoms such as excessive or prolonged menstrual bleeding, constant stomach pain, abdominal bloating, weakness or being excessively tired. All of the above may indicate fibroids, and if they are ignored, will result in severe blood loss and anaemia.
