 The American Oncology Institute (AOI) in Kanuru, Vijayawada, has achieved a significant milestone by successfully treating a massive retroperitoneal liposarcoma. Dr Vijay Koduru, Consultant Surgical Oncologist along with Dr Mrudula Tatakuri, Consultant Anaesthesiologist performed a laparotomy and surgical excision of a 30×20 cm retroperitoneal mass which lasted for about 7 hours.

The 64-year-old female patient was presented with persistent cough since a month. On clinical examination, the MRI scans revealed a huge 30×20 cm retroperitoneal mass encasing major vessels in the abdomen, which was challenging due to its anatomic location. Liposarcoma is a rare and aggressive form of cancer that arises in fat cells. Retroperitoneal liposarcomas are particularly challenging to treat due to their large size and proximity to vital organs in the abdominal cavity.

Sharing details of the case, Dr Vijay Koduru, Consultant Surgical Oncologist said, “Treating such huge retroperitoneal mass remains quiet challenging in this case due to associated major vessels attached to mass, diaphragmatic attachment, expected massive perioperative bleeding and need of prolonged ventilation. Intraoperatively, we noticed non recordable blood pressure for few minutes due to mass compressing Aorta and inferior vena cava. Inotropic support and blood transfusion were started intraoperatively. Postoperatively, patient required prolonged ventilation for a day due to massive intraoperative bleeding and high inotropic support. Patient recovered and was off ventilator on Day 2 but with newer complication – postoperative delirium was subsequently treated.

Retroperitoneal liposarcomas present unique challenges due to their size and location. The surgery was conducted with utmost precision, prioritizing patient safety and optimal outcomes. The patient got discharged on postoperative day 7 with no neurovascular deficits and   doing fine. The timely intervention of Perioperative surgeon, anaesthetist and intensivist the team successfully removed the mass. Patient has resumed to her daily routine life.
