A kidney transplant at Manipal Hospital Bhubaneswar brought
together medical expertise, careful planning and the selfless act of organ donation to give
66-year-old Mr. Nityananda Kar a new lease on life. Led by Dr. Manas Ranjan Pradhan,

Senior Consultant, Urologist and Kidney Transplant Specialist, the transplant team
successfully carried out a highly complex living donor kidney transplant using a kidney
donated by Mr. Pradip Kumar Sharma, despite several uncommon medical challenges
involving both the donor and the recipient.

What made this transplant particularly challenging was the unusual anatomy of the donors
kidneys. Instead of the usual one artery, one vein and one urine drainage tube in each
kidney, Mr. Pradip Kumar Sharma had two arteries, two veins and two separate urine

drainage systems in both kidneys. Such variations are extremely rare and make transplant
surgery far more demanding, as every blood vessel and urinary passage has to be carefully
preserved and reconnected to ensure the transplanted kidney functions normally.

Adding to the complexity, the donor kidney chosen for transplantation also had a small 4.5
mm kidney stone. After detailed evaluation and careful planning, the multidisciplinary
transplant team concluded that the kidney could still be safely transplanted while effectively
managing the additional surgical challenge posed by the stone.

The recipient, Mr. Nityananda Kar, had been living with end stage kidney disease and
required a transplant to regain normal kidney function. At 66 years of age, he also
represented a group of patients who are often considered high risk for transplantation.

However, after thorough medical assessment, the team found him to be a suitable
candidate. The transplanted kidney started working immediately after surgery, and Mr. Kar
recovered well with steadily improving kidney function, allowing him to look forward to a
healthier and more independent life.

Commenting on the case, Dr. Manas Ranjan Pradhan, Senior Consultant, Urologist and
Kidney Transplant Specialist, said, "This was a rare and challenging transplant as both the
donor and recipient presented unique complexities. The donor had multiple anatomical
variations and a small kidney stone, while the recipient was 66 years old. With careful

planning and surgical precision, the transplant was successful, and the kidney functioned
immediately. This case shows that neither complex anatomy nor age alone should prevent a
patient from receiving a life changing kidney transplant.quot;

Kidney transplantation remains the best treatment option for many patients with end stage
kidney disease. Compared to lifelong dialysis, it offers better long term survival, a
significantly improved quality of life and greater independence. This successful transplant

highlights how advances in surgical techniques, detailed planning and coordinated
multidisciplinary care are helping more patients benefit from transplantation, even when
faced with uncommon anatomical variations or older age.
