Successful Delivery After 3 Unsuccessful Pregnancies of a 37-Year-Old Woman

~The pregnant woman, who was dealing with Rheumatoid Arthritis and Hypertension, gave birth to a premature baby weighing 1.6 kg. The newborn girl needed NICU management for 35 days.

Mrs. Rinki Yadav, a 37-year-old woman, visited Wockhardt Hospitas, Mira Road with a history of Rheumatoid Arthritis, which she had been managing since 2015. She had experienced several miscarriages in the past. She got married on December 15, 2018, and got pregnant in September 2019, which ended in a miscarriage at 7 weeks with a spontaneous abortion. She immediately conceived again in November 2019, but unfortunately, this pregnancy also ended in a miscarriage at 7 weeks due to a Blighted Ovum. Her third pregnancy in April 2021 resulted in another miscarriage at 9 weeks, and further diagnosis revealed a Chromosomal abnormality, specifically Trisomy 21 (Down Syndrome)

In September 2023, Mrs Rinki Yadav conceived for the fourth time and because of High Risk Pregnancy, she was strictly monitored throughout the pregnancy at Wockhardt Hospitals, Mira Road. Unfortunately during this fourth pregnancy, she was diagnosed with Complete Placenta Previa and raised blood pressure, requiring her to be on bed rest throughout the duration. At around 30 weeks, she experienced heavy bleeding, prompting her to be admitted to Wockhardt Hospitals, Mira Road.

Complete Placenta Previa is a deadly disease for both the mother and the fetus. The risks of severe bleeding in pregnancy can be life-threatening to both mother and the baby There is a 3-4 fold rise in the chance of a newborn death. It affects 0.3-2% of pregnancies with a high risk of Preterm labor, Low birth weight, and complications at the time of birth

When she arrived at our Wockhardt Hospitals, Mira Road Emergency Department at around 2:30 a.m. on April 18, 2024, our on-duty doctors were prepared to attend to her, having been informed about her condition in advance. She was treated conservatively for 24 hours, where she was given steroid injections and magsulf infusion so that the baby would benefit from Lung maturity and prevent further Neurological complications. We conducted relevant investigations to assess the baby’s condition.

For two days, Mrs. Yadav was kept under complete observation and the baby was monitored. Given the severity of the patient’s condition and the potential risks to both her and the baby, the patient underwent a cesarean section. Dr. Mangala Patil (Consultant Obstetrician & Gynaecologist), Dr S N Agarwal (Consultant Senior Gynaecologist), and Dr. Nitu Mundhra (Consultant Pediatrician & Neonatologist) carried out the procedure. Throughout this process, we ensured that the patient and her family were fully informed about the situation and the potential outcomes.

Following the delivery, the baby’s weight was around 1.6Kg and cried immediately after birth. As the baby was born prematurely having other complications related to the same, she was admitted to the NICU for 35 days to receive the necessary care.
